Blood Pressure (From the JNC VII Guidelines)
Systolic: < 120 mm Hg -Normal
              120-139 mm Hg -Pre-hypertensive
              140-159 mm Hg -Hypertension (stage1)
              >160 mm Hg -Hypertension (stage2)

Diastolic: < 80 mm Hg-Normal
               80-89 mm Hg -Pre-hypertensive
               90-99 mm Hg -Hypertension (stage 1)
               >100 mm Hg -Hypertension (stage 2)

If the participant has diabetes and/or kidney disease the target range for systolic is <130 mm Hg and diastolic is <80 mm Hg.

Urine Albumin to Creatinine Ratio (ACR):

The following tests will be done on eGFRs below 60 only:

Calcium: 8.6-10.2 mg/dl -Normal
Phosphorus: 13-64 years: 2.5-4.5 mg/dl -Normal
Phosphorus: >64 years: 2.1-4.3 mg/dl -Normal
Intact PTH: 10-65 pg/dl -Normal

The following test will be done on participants with self reported diabetes OR glucose levels inidicative of diabetes only:

Hemoglobin A1c: ≤7% -Normal
Hemoglobin A1c: >7% -High
