Target Audience:
Designed for nephrologists, fellows, advanced practitioners, nurses, pharmacists, and clinic administrators.
Course Description:
This course is designed to offer the attendee a complete understanding of the optimal management of CKD and the needs for and operationalizing a CKD clinic. Management of various domains of CKD will be discussed. The practical aspects of designing and operating a CKD clinic will be reviewed. The attendee will get further insight into incorporating various innovative care models within the CKD clinic as well as utilizing technology in effectively delivering optimal care for CKD patients.
Course Objectives:
Upon completion of this course, participants will be able to:
- Realize the need for a CKD clinic in their practice domain matching local resources and utilizing guidelines to optimize it's efficiency
- Recognize some of the economic and business principles of setting up and monitoring a CKD clinic and optimize billing
- Implement best practices in the management of diabetes, hypertension, proteinuria and anemia, bone and mineral disorder of CKD.
- Recognize the importance of collaboration and providing multi-disciplinary resources in a CKD clinic
- Know what the patient expects from the CKD clinic
- Identify the benefits of adopting guidelines and implementing technology including automation and computerized clinical decision support system to help with guideline adoption and adherence and improve outcomes
- Use successful strategies to employ advanced practitioners to achieve optimal results in the care of CKD patients
