Target Audience:
This course is geared towards nephrology social workers and nurses.
Course Description:
Explore Transplant has partnered with ESRD Networks nationally and the NKF
on a quality improvement initiative to help improve transplant education in dialysis centers. To date, over 1000 dialysis providers have participated. Through this award winning program, dialysis providers will be trained how to help their patients make informed transplant choices. The program outlines the risks and benefits of transplant and living donation and helps patient decide whether transplant is personally right for them. Upon completion of this training, each participant will have increased transplant knowledge and will have designed a transplant education plan for his/her clinic(s) that supports the new CMS CfC Transplantation Regulations. In addition, each
participant will receive copies of the Explore Transplant education videos and print materials in English and Spanish for dissemination to their patients.
Course Objectives:
- Enhance your transplant knowledge.
- Gain experience delivering the Explore Transplant education program.
- Receive copies of the Explore Transplant education materials for dissemination to your
patients
- Design a transplant education plan for your clinic that supports the new CMS CfC
