The National Kidney Foundation’s Patient and Family Council is now accepting nominations for the fifth annual Wayne Nix Memorial Award. The award will be presented to a kidney patient who has demonstrated his or her commitment to improving the quality of life for others with kidney disease. The deadline for application submission is Monday, Aug. 15, 2005.
The award honors the commitment and life work of the late Wayne Nix, 1999 Chairman of the Patient and Family Council, a patient advocate and firm supporter of the National Kidney Foundation.
“Nominations can be submitted by anyone who is knowledgeable about the nominee’s commitment for raising awareness, supporting and educating others about kidney disease,” says Dr. Charles Schleifer, Patient and Family Council Chair. “Nominees who have a strong belief in rehabilitation and patient advocacy and exhibit this through current personal activities will also be considered. A recipient with chronic kidney disease can also be selected based on his or her inspirational story.”
Submissions must include a completed award application, two letters of support and at least two additional supporting documents detailing the nominee’s accomplishments.
