Cardio-Renal Care: An Integrated Best Practice Approach

This CME/CE program will include definitions, epidemiology, pathophysiology, and risk factors related to cardio-renal syndromes. An interactive case study will be presented using both the nephrology and cardiology perspective and then combining these for an integrated best practice approach to care.

From Silos to Teams: Tackling Risks for Cardiovascular Disease

This half–day, live CME/CE activity focuses on the benefits of a team approach to CVD risk management that incorporates the different inter-professional skill sets as well as available community resources to get more patients with risk factors to established goals set by evidence-based clinical practice guidelines.
