Program Overview
This half–day, live CME/CE activity focuses on the benefits of a team approach to CVD risk management that incorporates the different inter-professional skill sets as well as available community resources to get more patients with risk factors to established goals set by evidence-based clinical practice guidelines. A case-based approach will utilize “virtual patient experiences.” It utilizes a mnemonic of CVD risk: TOUCHDOWN! The term “touchdown” connotes a successful team approach and goal achievement. This activity leverages the positive perception of that term “TOUCHDOWN”, as well as assigns an easy to remember mnemonic for important CVD risk factors: Tobacco, Older age, Under-activity, Cholesterol, Hypertension, Diabetes, Over-Weight, and Nonadherence. Associated complications, such as diabetes and chronic kidney disease (CKD) are also discussed.
