Objective: Participants will understand the concept of disease management and the important role of the nephrology social worker in enhancing positive treatment outcomes related to quality of life, survival, adherence, patient satisfaction with care; identify the three phases of adjustment to progressive chronic kidney disease and the intervention goals of each phase, as well as specific interventions to enhance treatment outcomes. The NKF Council of Nephrology Social Workers, provider #1014, is approved as a provider for social work continuing education by the Association of Social Work Boards (ASWB), through the Approved Continuing Education (ACE) program, on a program specific basis.
The National Kidney Foundation is a Continuing Professional Education (CPE) Accredited Provider with the Commission on Dietetic Registration (CDR).
To further improve the knowledge and understanding of chronic kidney disease, NKF offers continuing education activities that are accredited for physicians, physician assistants, nurses, nurse practitioners, dietitians, pharmacists, social workers, transplant coordinators and dialysis technicians on an array of topics in varied formats for the user.
