Part 4 of the series Navigating Transplant, A Guide to Empowering Your Patients, focuses on the psychosocial and mental health dimensions of kidney transplantation. Covering the long span of the transplant experience, our expert presenter will demystify the components of the pre-transplant psychosocial assessment, discuss transitioning pre-transplant care needs to the independence that comes with being a recipient, reinforce the importance of educating and encouraging living donation and offer strategies on ways to talk with patients and care partners about the challenges and emotions integral to the transplant journey. This session takes place on Wednesday, April 24, 2024, 12 PM PT - 1 PM PT.

Through this event, we raised awareness about Chronic Kidney Disease (CKD) which affects 37 million Americans, and allowed many in Northern California affected with CKD to connect with others in their community.

Bay Area Ending Disparities in Chronic Kidney Disease Stakeholder Summit

Over the past few months, healthcare professionals and community members in the Bay Area have convened in four workgroups to discuss barriers, solutions, and equitable implementation strategies to improve early testing and diagnosis of CKD. In collaboration with these thought leaders. With nearly 100 participants in the summit, NKF and the summit workgroup members met on Wednesday, March 27th from 9:00 to 11:00 am PDT to share the Bay Area CKD strategy.
