Patient & Family Services

Crisis Intervention Fund

Unlike many of our programs that provide information or lend emotional and psychological support, the Crisis Intervention Fund channels money directly to patients and families who are experiencing a financial crisis due to kidney disease or transplantation.

Please contact your social worker at your dialysis clinic for an application or download an application (Get Acrobat 89kb).

Nutritional Supplements

Medicare, Medicaid, State Renal and private insurance programs typically will not pay for nutritional supplements, yet a good majority of patients need them and risk declining heath without them.

Please contact your social worker at your dialysis clinic for an application.

Patient/Family Symposium

The NKF Serving the Alleghenies’s annual Patient/Family Symposium is one of the key ways that we reach out and touch the lives of patients and families. Check back for more information.

Medical Alert Jewelry

Our Medical Alert Jewelry program is an important patient service that provides free-of-charge identification bracelets or necklaces to newly-diagnosed kidney disease patients or transplant recipients.

Please contact your social worker at your dialysis clinic for an application or download an application (Get Acrobat 51kb).

Transportation Program

Pennsylvania residents who meet eligibility requirements are reimbursed for a portion of the transportation costs associated with traveling to and from dialysis treatment three times a week. This program is run in collaboration with, and within the guidelines of, the Pennsylvania State Renal Disease Program.
