Help Participants Complete KEEP Healthy Survey

One of the most effective ways that we fight kidney disease together is through NKF's community-based KEEP Healthy program, which screens participants around the country for kidney disease. You can help to administer kidney health checks in your area and reach more Americans at-risk. Activities may include: Supporting the registration and greeting process, aiding participants in completion of health surveys, taking Body Mass Index (BMI), height, weight, blood pressure and other readings to assess risk.
Welcome each participant as they arrive at Station 2.
Even though participant’s can complete the KEEP Healthy Survey on their own,
participant’s should be asked if they would like to have someone complete the
Survey with them, or if they prefer to complete it on their own. Offering this
assistance helps reduce any anxiety for those who may be uncomfortable asking
for assistance, if they are unable to read the form for any reason.
If participant requests assistance:
Review each question on the survey with participant and document their
responses to the questions.
On the second page of the survey, check “interviewer administered” if the
participant did not complete the Survey on their own.
Direct all participants to Station 3 for their Body Mass Index (BMI) measurement.
Skill Sets: Courteous, helpful, patient, compassionate, and able to maintain composure
during busier times throughout the event. Flexible to assist with other KEEP Healthy
tasks, as needed.
Required training and/or credentials:
Participation in a NKF KEEP Healthy staff training required.
