The Award for Social Work in Transplant is given to a worker who has been in the nephrology social work field for two or more years and has made a valuable contribution to social work in the field of transplant renal social work.

Driven by a passion for patient advocacy and transplant care, Sloane Mosley, MSW, LCSWA is a post- transplant social worker with 11 years of experience working with chronic kidney disease patients, specializing in adult and pediatric transplantation. She provides comprehensive psychosocial support to patients and families as they navigate the complex medical, emotional, financial and social challenges associated with transplantation. 

Throughout her career, Sloane has contributed to the development of education materials, care transition resources and psychosocial support initiatives designed to improve post-transplant outcomes and patient engagement. Her efforts have helped streamline the transplant evaluation process, increase awareness and accessibility of available community resources and strengthen communication between patients, caregivers and transplant providers. 

In addition to her clinical work, Sloane serves as a field instructor for social work students, providing mentorship and practical training in healthcare and transplant social work. With more than 20 years of experience supporting children, adults and families across diverse practice settings, Sloane is passionate about advancing patient-centered care and improving the transplant experience. Her commitment to compassion, advocacy and collaboration continues to make a meaningful impact on the lives of patients and families throughout their transplant journey.
