This interactive workshop offers practical strategies to enhance CKD care through strengthened co-management among primary care, nephrology, and allied health professionals. Participants will discuss barriers and facilitators to GDMT implementation and effective co-management and will collaborate on identifying local solutions and workflow improvements to support coordinated CKD care
