Medicare coverage for dental care is extremely limited, especially for those on traditional Medicare. While some Medicare Advantage plans include dental services, traditional Medicare generally does not—unless the services are medically necessary for another covered procedure.
For years, the kidney community has been advocating for expanded dental benefits. People with chronic kidney disease (CKD), especially those with diabetes or on dialysis, are at greater risk for oral health issues, which in turn can worsen their overall health.
2025 Advocacy: Dental Care for People with Diabetic Kidney Disease
In February 2025, NKF and partners recommended that Medicare expand dental coverage to people with diabetic kidney disease (DKD) and diabetic retinopathy, conditions that are often linked to and made worse by poor oral health.
Research shows that treating periodontal (gum) disease can improve blood sugar control, reducing HbA1c levels and slowing kidney disease progression. Expanding dental benefits could support better outcomes for this high-risk group.
In May, NKF submitted a letter to the Trump Administration requesting that dental services be added to the Calendar Year (CY) 2026 Medicare Physician Fee Schedule (PFS) as covered services for people with diabetes and related kidney conditions.
Setback: Dental Coverage Not Included in Proposed 2026 Rule
Unfortunately, the PFS proposed rule released on July 14 did not include any dental coverage expansions, despite multiple public submissions. While this is disappointing, we understand that the Administration receives a large volume of policy requests, especially in the first year of a new term.

What’s Next?
We remain cautiously optimistic. NKF will continue engaging with CMS and members of Congress to push for expanded dental coverage for people with kidney disease. We plan to submit formal comments and recommendations before the September 2025 deadline.
We will also continue working with our legislative champions to advocate for this important change.
