Transplant Planning for Older Patients

Wednesday, September 24, 2025

Join us for this virtual session in the Navigating Transplant: A Guide to Empowering Your Patients series, focused on the power of collaboration between dialysis clinic professionals and kidney transplant teams.

This session explores the nuanced approach to transplant readiness in older, medically complex dialysis patients. Participants will gain a clear understanding of available transplant options—including the use of high KDPI kidneys, increased risk donors, and Hepatitis C-positive organs—and how these may expand access for appropriate candidates. Emphasis will be placed on best practices in patient counseling, supporting shared decision-making, and fostering effective coordination between dialysis and transplant centers.

This session will take place on Zoom on Sep. 24, 2025 at 12pm PT / 2pm CT / 3pm ET
