NKF provides opportunities for collaboration, networking, and career advancement for every member of the kidney patient care team. Tools and resources, based on NKF-published guidelines, are developed for professionals to incorporate into their practices and advance patient care. The National Kidney Foundation has the most extensive library of kidney health information for individuals, patients, transplant recipients, and living donors, including newsletters, printed brochures, online materials, and virtual workshops. Transportation Program
Pennsylvania Chronic Renal Disease Program – Transportation Reimbursement Program When a dialysis patient needs help with transportation costs to and from dialysis treatment, the Chronic Renal Disease Program (CRDP) may offer reimbursement to assist with these costs.
ABOUT THE PROGRAM:
As a participating Provider for the Pennsylvania Department of Health Chronic Renal Disease Program (CRDP) Transportation Program, the National Kidney Foundation administers reimbursement for public, shared-ride, invalid coach and non-emergency transportation expenses incurred by qualified dialysis patients in western and eastern Pennsylvania. Update: CRDP Income Limit Increase!
The income eligibility guidelines for the Chronic Renal Disease Program have increased to 325% or below the Federal Poverty Income Guidelines (FPIG) from 300% or below the FPIG. This means that for a single person, the income limit is now $44,168 and for a family of two, the income limit is now $59,508.
An application can be found at this link. Also, on the website under the Resources heading is the CRDP Patient Share Cost (PSC) Table and Federal Poverty Guidelines which will give you the income limits for larger families.
